Approval Process

Student Submits Accommodation Application

The student will submit the Accommodation Application to request reasonable accommodations. This includes a Medical Provider Form that will be filled out by an appropriate professional qualified to diagnose the disability.

Application is Reviewed by Accommodations Team

The Accommodations Team will review the application for completion and to determine the accommodation that Crown can reasonably provide.

Final Approval by Accommodations Team

Final approval will be determined by the Accommodations Team. The Accommodations Team will send approval/denial information to the student and what next steps are.

Student Schedules a Meeting With Accommodations Team For Follow-Up

Student Responsibilities:

  • Identify himself/herself as a student with a disability by completing the Accommodation Application to request reasonable accommodations.
  • Provide adequate documentation of the specific disability. Documentation must be signed by an appropriate professional qualified to diagnose the disability.
  • Follow all applicable Crown College policies and procedures.
  • Notify the Accommodations Team if there are any changes to the student’s diagnosis or accommodation needs.
  • For non-academic accommodations, students must re-apply each academic year.
  • For academic accommodations, in each new term, or in the event a student has new professors, the student must complete the Academic Renewal process. This includes submitting any updated documentation, uploading their current class schedule, and providing consent for the Director of Disability Services to share accommodation information with their new professors.
